Cardiac System

Robert William is a patient currently in the hospital. He suffers from COPD and Congestive Heart Failure.

Inpatient Hospital Room

1. Identify normal and abnormal cardiac assessment findings, including heart rate, rhythm, and heart sounds.
2. Perform a focused cardiac assessment, including inspection, palpation, and auscultation of the precordium and peripheral pulses.
3. Assess for signs of cardiac compromise, such as edema, capillary refill, jugular vein distension, and skin temperature.
4. Measure and interpret vital signs, including apical pulse and blood pressure, using manual and electronic methods.
5. Demonstrate effective communication techniques while explaining procedures and maintaining patient comfort.
6. Accurately document assessment findings using appropriate nursing terminology and formats.

1. Demonstrate correct technique for auscultating heart sounds (S1, S2) and locating the point of maximal impulse (PMI).
2. Identify the presence or absence of extra heart sounds or murmurs, if applicable.
3. Assess peripheral circulation by checking capillary refill, peripheral pulses, and skin color and temperature.
4. Communicate clearly with the patient during the assessment, promoting relaxation and cooperation.
5. Document cardiac assessment findings using proper structure (e.g., narrative notes or head-to-toe flow sheets).
6. Verbalize when to notify the instructor or provider of abnormal findings.
