Learning Objectives

  1. Conduct a comprehensive assessment of a patient with CKD, including fluid status, electrolyte balance, and lab value interpretation
  2. Recognize signs of CKD-related complications, such as hyperkalemia, fluid overload, and anemia.
  3. Administer medications safely, considering renal dosing and nephrotoxic risks.
  4. Monitor for and respond to critical lab results
  5. Educate the patient on dietary restrictions, fluid management, medication adherence, and dialysis preparation if applicable.

Competencies

  1. Identify and interpret abnormal renal labs and assess for clinical symptoms of CKD complications.
  2. Accurately administer medications that are renal-safe and understand those that must be avoided or adjusted.
  3. Implement interventions to address fluid overload, electrolyte imbalances, and hypertension.
  4. Communicate lab abnormalities to the healthcare provider using a structured format such as SBAR.
  5. Provide patient teaching regarding diet (low sodium, potassium, phosphorus), medication management, and future dialysis needs.
  6. Document assessments, interventions, and patient responses in the electronic medical record clearly and accurately.
