Robert Williams is a patient currently in the hospital. He suffers from COPD. He recently was admitted into the hospital due to a CVA.

Learning Objectives

  1. Identify the components of a basic neurological assessment, including level of consciousness (LOC), orientation, pupil response, motor strength, and sensation.
  2. Perform a focused neurological assessment using inspection, simple commands, and basic physical exam techniques.
  3. Assess and document level of consciousness and orientation, using tools such as Glasgow Coma Scale or AVPU.
  4. Evaluate motor function and strength in upper and lower extremities bilaterally.
  5. Assess pupil size, symmetry, and reactivity to light using a penlight.
  6. Communicate clearly with the patient during the assessment, explaining each step and using age-appropriate language.
  7. Document all assessment findings accurately, noting both normal and abnormal results.

Competencies

  1. Demonstrate accurate technique for assessing level of consciousness, orientation, and response to verbal stimuli.
  2. Assess and document pupil size and reactivity using the PERRLA mnemonic (Pupils Equal, Round, Reactive to Light and Accommodation).
  3. Evaluate and document bilateral motor strength and ability to follow simple commands (e.g., grip strength, leg lifts).
  4. Identify and report abnormal findings (e.g., unequal pupils, weakness, confusion) to the instructor or provider.
  5. Use therapeutic communication throughout the assessment to reduce anxiety and promote cooperation.
  6. Record neurological findings using proper clinical language in narrative or checklist format.
