Postpartum Hemorrhage

Darla Johnson is a 30 year old female who just delivered a healthy newborn. She is recovering from her delivery just a few hours ago. She is starting to feel very exhausted and light-headed.

Inpatient Hospital Room

1. Recognize early signs of postpartum hemorrhage
2. Perform an urgent focused postpartum assessment, including fundal checks, lochia evaluation, vital signs, and bladder status.
3. Initiate emergency interventions, such as fundal massage, notifying the provider, and preparing for uterotonic administration.
4. Use structured communication techniques (e.g., SBAR) to escalate care and activate rapid response if needed.
5. Provide emotional support to the patient and family during a high-stress obstetric emergency.

1. Identify and document signs of postpartum hemorrhage early and accurately.
2. Perform fundal massage and assess uterine tone and bleeding in response to intervention.
3. Administer uterotonic medications per standing orders or provider instruction and monitor for side effects.
4. Maintain hemodynamic stability through IV fluids and blood product support, if indicated.
5. Provide updates to the patient and support person using clear, calm communication.
6. Document all interventions, assessments, and outcomes accurately in the EMR.
